Separate observations from explanations. Describe the task, recent changes and circumstances. Select one manageable support action; bring unexplained or concerning changes to a health professional. The NOTICE → UNDERSTAND → CHOOSE → ACT → REVIEW → ADAPT framework organizes decisions; it does not diagnose illness.

30-DAY OBSERVATION
A clear baseline, one completed support action and notes on whether the task became easier, unchanged or harder.
ILLUSTRATIVE CASE
An older reader values visiting a friend but finds the journey difficult. Checking accessible transport creates a practical option. It does not establish why the difficulty developed.
CARE CHECK
Keep changes in function visible. Persistent or worsening problems deserve professional assessment.
ACTION CHECKLIST
Choose one relevant need → record observations → choose a suitable action or care contact → act → review → adapt. Seek care sooner whenever symptoms or your care plan require it.
